Phil Jagielka could be out until Christmas after undergoing a second operation on his knee.
Everton manager David Moyes revealed the defender had surgery on Wednesday to repair a small cartilage tear.
The 27-year-old has been sidelined since damaging knee ligaments against Manchester City in April but had been due to return to light training this week.
"Jagielka had an operation on a tear in his meniscus, which is the cartilage in his knee," said Moyes.
"We think [it has set him back] four to six weeks. He was down initially but he's on the road to recovery now."
Moyes also revealed a further setback for striker Victor Anichebe, who has been missing since suffering a similar injury to Jagielka's against Newcastle United in February.
The Nigerian looked to be nearing a return following the removal of a pin from his knee last month but an adverse reaction to the procedure has left the Everton manager uncertain when he will be able to call on the 21-year-old again.
"Victor's not in a good way either," said Moyes. "He had the pin taken out four to six weeks ago which had been catching on the side of his knee.
"Now what has happened (is that) he has a massive growth of bone and has a really big knee at the moment. We don't know how long it will be."

wow! whats with all the negativity?? yes we have lots of injuries but were only 6 points off villa in 5th with a game in hand! win that and were into the european positions - hardly the time to give up on the season and feel sorry for ourselves. we are in a strong position in the europa league and still have the fa cup to go for. lets have some positivity!! keep it up moyes and squad, the injured will be back for the most important part of the season :-)

Why iseverybody talking about the relegation zone and trying to stay up? Even without Jags, Mikky, Vic, Peinaar etc. we are still good enough to be at least top half - the players who have been playing the last couple of months are all good players who should be winning premier league games. Its far to early to start talking about writing off the season, we're not even past xmas yet! I think we should still be looking at a top 8 finish this season and we definitely should still be aiming for Euro qualification still - not looking the other way.

Injuries have made this season a wash out. It's a shame cos when all are fit we have a fantastic squad of players. If we can hang on in the europa league til after xmas and get some players back, we should focus on europe, the fa cup and staying up. If we can get players back for next season we can then have a real go at the premier league.

Grrrrr! At least I've got used to this feeling now of learning about these setbacks. Why hasn't there been a post about Anichebe's setback? He's not as important as Mikel or Jags, but he'd be a useful sub. Everton without Mikel and Jags are like Chelsea without Lampard and Terry, Arsenal without Fabregas and Gallas/Vermaelen, Liverpool without Gerrard and Carragher (I could go on!), not to metion we've been without Pienaar, Osman, Billy, Neville and Saha in recent weeks although hopefully we'll have some back for the Utd game. Great yet strange to have that winning feeling back (7 losing games since we beat BATE) and I'm really proud of the players who have kept us above the relegation zone in recent weeks. COYB!

I don't like to be negative about my club, but seriously, we can forget about this season - need to build for next season now, and just make sure we doe enough to stay up this season! COYB - lets start by having a decent performance on sunday. We should be beating west ham even if the likes of Mikel and Phil are out injured. Desperately need you two back - We can't afford to have our key players out this long ! :-(

I knew there was something up with Jags and Mikel because i always look at the injuries section of this website and both their expected comebacks showed late 2009/early 2010. This is devasting news when i first read it on teletext, our spine of our team hasnt played together since February, thank god we have a 1st class goalkeeper in Tim because we would be in a worse situation. Put in to the mix that peanuts and saha are not fit
