David Moyes has made just one change for his final game in charge of Everton as Steven Naismith comes in for the injured Leon Osman.

As expected, skipper Phil Neville misses the match due to a knee problem, meaning his Everton career has come to a close.
Victory at Chelsea's Stamford Bridge could see the Blues rack up a record Premier League haul of 66 points.

The hosts, meanwhile, could need a win to ensure a third-place finish and automatic qualification to the Champions League group phase.

Naismith's inclusion means he makes his first start since a 2-1 defeat at Norwich City in February.

Tony Hibbert, who featured in Moyes' first XI as Goodison chief in March 2002, is named on the bench.

The right-back slot will instead be occupied once again by Seamus Coleman, who joins Phil Jagielka, Sylvain Distin and Player of the Season Leighton Baines in Tim Howard's defence.

Naismith, meanwhile, is expected to play out of a midfield also comprising Steven Pienaar, Darron Gibson, Marouane Fellaini and Kevin Mirallas.

Victor Anichebe starts in attack ahead of Croatian Nikica Jelavic.

Neville's absence means he ends his Everton career having made a total of 303 appearances.
